so you start mplayer and it is jerky, pausing, or pixelated?
It also automatically shifts magically to “the color scheme has been changed to windows 7 basic”?
Turns out for me (windows 7 64 bit) the fix was to specify that it use “-vo direct3d” (smplayer has an option for it, too) then it worked fine.
Also updating my graphics drivers for my nvidia card fixed the problem, as well (GE 6150 is what I have).

fix: update my libdvdcss version (or install it at all), the bundled one was what was causing it, for apparently being too old (1.2.10 doesn’t cut it I guess).
The other “fix” I’ve had success with in the past is to (see it fail in mplayer then) play the DVD once in VLC, then try it again in mplayer, and poof, magically it works. Odd.

I think all of these meant “you are skipping straight to a title in mplayer, like dvdnav://1 which sometimes fails, whereas dvdnav:// works ok, as well as dvd://”
